People Who Eat Fermented Foods Often Show Lower Mortality Risk? A Meta-Analysis Using Data From Over 3 Million
“Fermented foods are good for your health.”
We hear this all the time. But does that really mean people who eat fermented foods regularly are more likely to live longer? A new study tackled this straightforward question with large-scale data.
In February 2026, a meta-analysis examining the relationship between fermented food consumption and mortality risk was published in the academic journal Frontiers in Nutrition (DOI: 10.3389/fnut.2026.1714437). This is the first comprehensive meta-analysis of its kind, integrating and analyzing 50 cohort studies that included over 3 million participants.
To cut to the chase: people who frequently eat fermented dairy products like yogurt and cheese showed a “tendency” toward lower mortality risk.
But here’s something I want to emphasize strongly. This research demonstrates “association (correlation)” — it does not prove a “causal relationship” that says “eating fermented foods will help you live longer.” This distinction is incredibly important.

What Is a Meta-Analysis?
Let me start by explaining what a meta-analysis is.
A meta-analysis is a method that gathers multiple already-published research results and statistically combines them. Individual studies can sometimes be small in scale or produce varying results. By pooling them together, we can paint a more reliable picture of the bigger story.
This research combined 50 “prospective cohort studies” from around the world. A prospective cohort study follows many people over an extended period and examines the relationship between lifestyle habits (in this case, fermented food consumption) and subsequent health outcomes (in this case, mortality).
The combined total of over 3 million participants is exceptionally large for a nutritional epidemiology study. That scale makes it much easier to spot overall trends.
The “Lower Risk Tendency” Seen With Fermented Dairy and Cheese
The study analyzed mortality risk associations for different types of fermented foods. Let’s look at the main findings.
Fermented dairy products (including yogurt) showed a tendency for lower mortality risk among frequent consumers.
- All-cause mortality (death from any cause): relative risk approximately 0.94
- Mortality from cardiovascular disease: approximately 0.93
- Mortality from cancer: approximately 0.94
A relative risk of 0.94 means “the group that consumed the most had about a 6% lower risk compared to the group that consumed the least.” It’s not a dramatic difference, but across a 3-million-person scale it was a statistically meaningful association.
Cheese also showed a lower tendency for all-cause mortality (relative risk about 0.97). For cardiovascular death, no clear association was found, though lung cancer showed a protective association.
Chocolate (which undergoes fermentation in cocoa production) showed lower tendencies for both all-cause mortality (about 0.90) and cardiovascular mortality (about 0.84). However, this doesn’t mean we should eat lots of chocolate. Commercial chocolate often contains significant amounts of sugar and fat, so we need to be careful about consumption.

Miso: “No Significant Association” — An Honest Result
Here’s what caught my attention as a Japanese person: what about miso?
The data on miso is limited, and interpreting the results requires caution. While some studies reported an association with lower all-cause mortality, no clear significant associations were found for cause-specific deaths. Overall, the paper suggests we still need to be careful about drawing conclusions about miso.
It’s easy to think “fermented foods = universally good for you,” but when you look at the data, results vary significantly depending on the food. The inconsistent miso results likely stem from various factors — the high sodium content in miso, differences in study populations, variations in intake amounts, and more.
I think there’s real scientific integrity in honestly reporting results that didn’t show clear effects. Not cherry-picking only the favorable findings — that’s what builds trust.
Bread, too, showed no significant associations with either all-cause mortality or cause-specific mortality.

“Association” Versus “Causation” — The Most Important Point
The most crucial thing to understand when reading this study is this:
What this meta-analysis demonstrated is only “association (correlation)”. The observation that “people who eat fermented dairy frequently tend to have lower mortality risk” is real. But it doesn’t mean “causation” — that eating fermented dairy actually caused the lower risk.
This is because observational studies have a pitfall called “confounding.”
For example, people who eat yogurt frequently might simply be more health-conscious to begin with. They might eat lots of vegetables, exercise regularly, and not smoke. In that case, the lower mortality risk might be due to “their overall healthy lifestyle” rather than “yogurt specifically.”
The research team openly acknowledges this. The paper lists these limitations:
- Dietary records are self-reported and can contain errors
- As an observational study, causation cannot be established
- It’s difficult to standardize the types, manufacturing methods, and quantities of fermented foods
- Not all confounding factors like overall diet quality can be perfectly adjusted for
In other words, this study isn’t declaring “eat fermented foods and you’ll live longer.” Rather, it’s a study that says “we carefully examined the association between fermented food consumption and health using large-scale data.”
Why This Research Still Has Value
You might feel disappointed hearing “we can’t determine causation.” But this research genuinely matters.
The fact that such a consistent association between fermented dairy consumption and lower mortality risk appeared across this much large-scale data suggests it’s worth pursuing more targeted research (like intervention trials). It provides a solid foundation for testing a hypothesis.
For our daily eating habits, too, this research offers a useful reference point. Incorporating moderate amounts of fermented dairy into meals becomes one option to consider when thinking about food choices.
But again, to repeat myself: this isn’t saying “eat lots of one specific food and you’ll become healthy.” The most realistic approach — and the one without strain — is to enjoy fermented foods as part of a balanced diet overall.
How to Approach Fermented Foods
This meta-analysis showed us the largest-scale “big picture” currently available about the relationship between fermented foods and health.
Fermented dairy products like yogurt and cheese showed a tendency to be associated with lower mortality risk. Meanwhile, foods like miso showed no significant differences. It became clear again that “fermented foods” is a broad category with diverse contents and varied relationships with health.
What matters is not taking these research results to mean “we found a magic food,” but rather approaching them calmly as “one hint for reviewing our eating habits.” Fermented foods enrich our daily tables. We can treasure that joy while quietly keeping an eye on the bigger picture that science reveals.
The numbers aren’t saying “eat this and live longer.” And yet, they quietly show a hint that our daily tables and our health are somehow, somewhere, connected.

Reading this research gave me a chance to think more carefully about how I approach fermented foods.
When I hear “fermented foods are good for you,” it’s easy to jump to a simple conclusion: “eating them will make me healthier” or “I should eat more of them.” I myself have felt a kind of reassuring sense about familiar fermented foods like miso, yogurt, and natto. But when I look at research like this — studies that gather large amounts of data — I realize that even though they’re all called “fermented foods,” the results vary quite a bit depending on the specific food.
Fermented dairy products like yogurt and cheese showed a tendency to be associated with lower mortality risk. Yet miso and bread didn’t show clearly significant associations in some areas. That result was a bit surprising to me as someone who’s always felt close to miso as a Japanese person.
But I don’t think that means “miso is pointless.” Miso has its own value. Drinking warm miso soup feels grounding. It makes meals more satisfying. It makes it easier to eat vegetables and tofu together. These roles in daily meals are things that a single number like mortality risk can’t fully measure.
At the same time, I’ve come to realize it’s also wrong to think “as long as I eat miso, I’m safe” or “fermented foods are all good for you.” Miso contains salt. Some yogurts have lots of added sugar. Cheese, depending on how you eat it, can mean high fat or sodium intake. We shouldn’t feel safe just because something has the label “fermented food.” What matters is what we eat, how much, and in what kind of overall diet.
What struck me as most important in this article was the distinction between “association” and “causation.” Even if people who eat fermented dairy frequently show a tendency toward lower mortality risk, that doesn’t mean “eating fermented dairy actually caused them to live longer.” People who eat yogurt often might already have high health awareness, exercise regularly, eat lots of vegetables, or have other healthy habits.
In other words, it’s very difficult to isolate a single food and say “this is what worked.” Eating is something that accumulates day after day, and it’s connected to sleep, exercise, stress, age, living environment — many things.
That said, I don’t think this research is meaningless. In fact, I believe the tendencies emerging from data on over 3 million people have real value. Rather than rushing to conclusions from a single study, we gather many studies and look at the whole picture. Simply learning that “it seems worth investigating the relationship between fermented foods and health in more detail” is already an important step.
For me, fermented foods aren’t a special health regimen — they’re part of my daily table. Eating yogurt in the morning. Having miso soup. Adding a little pickle. These small habits might not be flashy, but they seem to help set the rhythm of my daily life.
But I also want to keep that feeling separate from scientific results. “I feel better when I eat this” — that sense is important. But if we extend that to say “everyone will benefit the same way,” that becomes a bit risky. Bodies and lives are different from person to person. That’s why when I talk about fermented foods, I want to do so without being dogmatic or pushy, keeping a healthy distance.
This research didn’t elevate fermented foods to something magical. Rather, it taught us that results differ by food, that research has limits, and that what we observed is only an association. I actually find that honesty trustworthy.
Health information tends to get pulled toward simple language. “Eat this and you’ll be healthy.” “This means a longer life.” Those phrases are easy to remember. But actual bodies and diets are far more complex. That’s exactly why, even if it’s a bit cumbersome, it matters to look into the actual research, to see what we can and can’t really conclude.
Fermented foods have gradually become rooted in how people live over long periods of time. Miso, soy sauce, yogurt, cheese — each has been eaten continually in its own land and culture. Behind that are values that health effects alone can’t explain: preservability, taste, ease of eating, how naturally it fits on the table.
I plan to keep enjoying fermented foods without strain. Rather than getting swept up in research results, I want to care for my diet as a whole, fitting fermented foods naturally into it. Some days miso soup, some days yogurt. A bit of cheese sometimes. This easy-going way of approaching it seems to fit me.
The numbers aren’t saying “eat this and you’ll live longer.” But they’re showing us the possibility that what accumulates on our tables connects somewhere with health. I can avoid being overconfident about that connection, without ignoring it either, and keep living with fermented foods.
Maybe fermented foods aren’t meant to push us toward quick answers. Maybe they’re meant to be slowly savored and reflected on, little by little, in the midst of everyday life.
